Jeanne Lambrecht shall not assume responsibility or be held liable for any injury to person, damages or loss of property, suffered by any student for any reason. The undersigned is familiar with the risks and perils inherent in sports activities; specifically equestrian activities. Further the undersigned is aware of the risk of personal injury to his/her child when undertaking such sports activities. Therefore, in consideration of being permitted to participate in pitching instruction, the undersigned hereby voluntarily assumes all risks of personal injury, property damage, and/or other damages to the undersigned's child resulting from or in any way associated with his/her participation in pitching instruction.

Further, the undersigned hereby releases Jeanne Lambrecht and the owner of any facility used during instruction from every claim, liability or demand of any kind or on account of any personal injury, property damage, or other damages resulting from or in any way associated with the child of the undersigned's participation in said sports activities. The undersigned also assumes responsibility for any property damaged as the result of malicious intent or gross negligence. The undersigned confirms he/she has read and understands this release and is at least 18 years of age.
